Position Summary: The Southwest Minnesota region of Mayo Clinic Health System provides chronic, urgent and preventative services. Responsibilities: The interdisciplinary Palliative Medicine team works to reduce the burdens associated with life limiting illness by enabling people to have the best quality of life possible throughout the course of illness. The Palliative Medicine PA/NP provides expert management of symptoms that contribute to the suffering of patients and their loved ones. The Palliative Medicine clinician provides assistance in complex medical decision making via goals of care discussion. These discussions personalize care to patient values. Together, the clinician along with the Palliative Medicine team addresses physical, psychological, spiritual needs of patients and loved ones.

Services Provided/Responsibilities:

* The NP/PA will consult with the patient and their loved ones to help delineate their goals of care and aid in complex medical decision making.

* The clinician will coordinate to the patient's plan of care and informs primary care physician of recommendations.

* The clinician will assist in the department of clinical practice guidelines/standards in support of quality Palliative Medicine as directed by supervisor.

* Promotes collaborative relationships between medicine, nursing, and other healthcare processionals to ensure whole person care.

* Discuss advanced care planning with patients and families.

* Optimized symptom control (pain, nausea, dyspnea, etc.).

* Educate patients and family to promote understanding of underlying disease process and expected future course of the illness.

* Assist actively dying patient and their families in preparing for life closure.

* Educate and mentor staff/students/ medical residents.

* Promote a system of care that fosters timely access to Palliative Medicine services.

Qualifications: Nurse Practitioner - Master's degree in nursing required, Doctorate of Nursing Practice (DNP) preferred or Clinical Nurse Specialist - Master's degree in nursing required or Physician Assistant - requires completion of a baccalaureate degree and an accredited physician assistant educational program, Master's degree preferred.

Certification by a national nurse certification organization acceptable to the Minnesota Board of Nursing required. Physician Assistant (PA) certification by the National Commission on Certification of Physician Assistants.

Additional Qualifications: Experience with complex symptom management and working with the seriously ill patient preferred.

License or Certification: Current license or ability to obtain license to practice in the State of Minnesota. Currently possess or ability to obtain DEA. Basic Life Support (BLS) certification required.

Compensation Detail: Education, experience and tenure may be considered along with internal equity when job offers are extended.

Schedule Details: Monday - Friday, days. Anticipated future holiday/weekend coverage. Growing practice with plans of expansion into the outpatient setting. Potential for outreach services/call coverage
