About this job

performs concurrent and retrospective clinical chart reviews and abstracts data for the purpose of reporting clinical quality measures and risk assessments. The information will be entered into the required electronic assessments/forms to support LPG quality initiatives (HEDIS, Medicare Star, etc.) and pay for performance or shared savings initiatives. This position provides indirect assistance to the physician, patient and care team by supporting patient adherence to the medical plan of care and/or evidenced based medicine guidelines to include all appropriate preventative and disease-specific screenings, lab/diagnostic testing and scheduled follow up appointments. Communicates with this population of patients via telephone, MyChart, or the preferred method of correspondence to support the patient adherence to the plan of care and/or evidenced based medicine guidelines and documents their intervention in the electronic health record.

Requirements:

Education Requirements: High school diploma, GED. Associates/ Occupational Associates Degree in Medical Assisting or Diploma from Vocational Medical Assisting Program preferred.

Experience Requirements: 3 years (fulltime) recent clinical experience in Internal Medicine and/or Family Practice utilizing an electronic health record required.

Certifications/Registration Requirements: Medical Assisting credential through AAMA, AMT, NCCT or NHA required within 2 years of hire.

Other: Computer proficiency in Microsoft applications (Word, Excel) required. AHA BLS required (must be obtained within first 90 days of hire. Early/late hours and/or weekends may be required.
