CLIENT CARE MANAGER

 America’s #1 provider of non-medical home care for seniors seeks a professional and caring individual to provide the highest quality of care. Home Instead Senior care  is recognized in 2017 on Forbes best Large Employer's List. The Position involves working directly with seniors and  family members, as well as case managers and other care providers. Candidate must have the ability to work independently in a fast pace setting. 

 
The Client Care Manager is expected to perform a variety of duties that relate to client care including care consultations with potential clients and family members, client/CAREGiver introductions and quality assurance visits with existing clients. They continuously evaluate the plan through a series of ongoing communications and visits to ensure high quality of care, client satisfaction and retention. They oversee a State contract providing services to qualified clients. 

PRIMARY RESPONSIBILITIES
 
    -    Reflect the core values of RMB Group, LLC, (d.b.a. an independently owned and operated  Home Instead Senior Care franchise)
    -    Knowledge of the senior care industry or related fields
    -    Take incoming calls and take action on, service inquiries, client and caregiver concerns with   a sense of urgency
    -    Meet with potential clients and family members to discuss their needs and provide solutions, which includes preparing a Plan of Care
    -    Conduct Service inquiries,Care Consultations and CAREGiver Introductions as needed
    -    Conduct follow-up visits as necessary providing solutions to various issues that may arise to ensure clients are receiving services needed
    -    Maintain and update all forms necessary for client care
    -    Monitor, mediate, and log client  activity, modify Plans of Care as needed
    -    Monitor and audit all compliance documents within regulations and standards guidelines


EDUCATION/EXPERIENCE REQUIREMENTS
   
-    A Bachelors or Associates Degree in a related Field (Health Care, Allied Health, Social Service, Geriatrics) preferred
    -    Compassionate and sincere interest in caring for seniors
    -     Must  be able to work evenings and weekends as required as well as one mandatory on call weekend per month
    -    Excellent written and verbal communication skills with the ability to write reports and proposals
    -    Excellent interpersonal and Telephone etiquette skills
    -    Must have a valid Drivers License 
    -    Must have a vehicle in good working order, suitable for daily travel
    -    Experience in a Client Care or Case Manager Position preferred
    -    Strong Computer Skills
    -    Strong written and verbal communication skills
    -    Strong Problem Solving Skills with the ability to deescalate client or caregiver situations when appropriate
    -    Knowledge of Managed care industry, specifically Long Term Care and State Community Based Programs is a plus

HOURS: M-F 8:30-4:30
