Description:

The Clinical Nurse is a competent registerednurse in this institution and will deliver nursing care to patients of varyingage groups by utilizing the nursing process and nursing practice standards. TheClinical Nurses use Nursing Practice Competencies to intervene to achieve safe,optimal patient and family centered care health outcomes; LeadershipCompetencies to coordinate patient care and communicate with the Care Team;Professional Profile Competencies to develop professionally as a life-longlearner, and Practice Outcomes Competencies to evaluate care based on evidence,the ULH quality model, and patient safety goals. 

The Clinical Nurse demonstrates competency caring for complex patients andparticipates on the Care Team to improve the quality of care in the clinicalarea. 

POSITION ACCOUNTABILITIES

1. Conducts systematic, continuous and comprehensive assessments of patienthealth status from admission or encounter to discharge across the carecontinuum to ensure patient safety, collaborating with the healthcare team,patient and family. Care addresses all aspects of the individual and family,including age, gender, spiritual, cultural, physical, psychosocial and patientpreferences. Proactively uses safety surveillance to prevent adverseoutcomes. 

2. Independently administers medications per licensure authority, assesses andreassesses the effects of these medications, and documents administration in atimely manner per policies, procedures and regulatory requirements.Consistently adheres to patient safety guidelines regarding all aspects ofmedication administration. 

3. In collaboration with patient and family, plans care using currentscientific knowledge, theory, and care standards within the nursinginterventions and outcomes framework and national patient safety goals. Constructsindividualized, age specific plans for optimal patient and family outcomes, asappropriate to the practice setting. Incorporates data from themulti-disciplinary Care Team into the plan of care to assure continuity andappropriate patient teaching. 

4. In collaboration with patient and family, identifies and prioritizes patientproblems based on subjective and objective assessment data, patient safetyconsiderations, patient and family goals and preferences, and nursinginterventions and the medical plan of care. 

5. Implements nursing care interventions to meet patient and family centerhealth care needs. Demonstrates competent skills, appropriate clinicaljudgments and chain of command to promote patient safety and reach planned caregoals and outcomes. 

6. Competently and consistently assesses, conducts and evaluates the outcomesof patient and family teaching, from admission, or episode of care, todischarge. Teaching reflects sensitivity to age, learning needs and culturalbackground. 

7. Evaluates care by performing regular and systematic reviews of patient/family responses to interventions; revises the multi-disciplinary plan of carein collaboration with other care Team members to promote desired outcomes.Recognizes care variances and takes steps to prevent or intervene withpotential or real adverse events to promote safe care. 

8. Serves as an advocate for the patient and family, demonstrates respect fortheir rights, responsibilities, and sensitivity to their preferences. Preparesthe patient and family for discharge; obtains feedback to promote positivepatient satisfaction outcomes.

1. Exercises informed judgment at the competent level of practice, assumingresponsibility and accountability for patient care assignments, demonstratingappropriate priorities for care and commitment to patient, family, and the CareTeam.

2. Works collaboratively, with appropriate monitoring and delegation, withunlicensed, assertive healthcare providers, per regulatory and legalrequirements, to assure safe patient care delivery.

3. Actively participates as a collaborative member of the healthcare team bycommunicating constructively using core behavioral standards. 4. May act in the role of relief Charge Nurse when the unit need arises.

3. Evaluates patient and family care in relation to scientific knowledge,identifying and participating in improvement using hospital quality controlmodel.

4. This position requires the employee to be responsible for the assessment,treatment, and care of patients of certain age categories, as appropriate toassigned patient populations. 

5. Participates in performance improvement activities; quality improvement andpatient safety activities; assists in maintaining compliance with JCAHOaccreditation and other internal and external regulatory standards includingthe code of conduct. 

6. Participates in Magnet activities. Active participant in the Family CenterCare model.

Qualification:

EDUCATION\: 
BSN preferred. Will consider ADN candidates witha minimum of two years hospital care experience.A minimum of an AssociateDegree in Nursing from an accredited nursing program is required.  ABachelor’s Degree in Nursing from an accredited nursing program ispreferred.  Effective May 1, 2014, all new-hire Associate DegreeRegistered Nurses (new graduate and experienced) are required to start anapproved BSN program within two years of hire and complete an approved BSNprogram within five years of hire. 

 

EXPERIENCE\: 
Candidates with a BSN\: minimum one year ofhospital patient care experience preferred. Will consider BSN new graduatesdepending on unit specific needs

Candidates with an ADN\: minimum of two years ofhospital patient care experience required. Must have 1 year experience toqualify for PRN positions.

 

LICENSE & CERTIFICATES\: 
Experienced BSN and ADN candidates\: KY RNlicense required. Current AHA certification in Basic Life Support (BLS).Specific clinical areas may require additional experience or certificates suchas ACLS, PALS or others as identified.

BSN new graduates\: KY RN provisional licenseand/or KY RN license required. Current AHA certification in Basic Life Support(BLS). All new BSN graduates will participate in the Nurse Residency programand successfully complete all activities as required.
