About this job

The Manager of Care Coordination is responsible for the oversight of Care Coordination including, but not limited to: clinical resource management, discharge planning activities, patient advocacy, clinical social work and best practice in medical necessity determination and concurrent review.

This position will oversee the department in an efficient manner to ensure timely and compliant care coordination, discharge planning, utilization review and social work interventions resulting in quality patient care. The Manager consults and collaborates with other managers, departments, physicians, administration and healthcare workers regarding departmental activities or initiatives. This position leads or participates in quality and process improvement projects related to care coordination functions. The Manager is responsible for all performance management activities, including hiring, training or disciplinary issues. This position will conduct, lead, facilitate or participate in staff meetings, interdepartmental meetings, post-acute care provider meetings or patient/family meetings as appropriate. The manager will develop or participate in the development of the departmental operating budget, and monitor scheduling and expenditures to verify that the department is operating within the established business plan. This position reviews documentation or utilization data for the purposes of reporting, trending, quality improvement or validation of compliance as indicated. The Manager guides department operations according to the organizational objectives, hospital policies, standards of practice and Federal and State Regulations.

Requirements:

* Comprehensive knowledge of care management and discharge planning, Medicare and Medicaid admission and review requirements, and general commercial admission and review requirements.

* Minimum of 3 years in a Care Coordination or Case Manager role.

* Working knowledge of Interqual criteria

* Effective communication skills, especially with medical staff.

* Working knowledge of finance and budgetary process, and government billing regulations. Knowledge of Conditions of Participation and Joint Commission standards.

* Current manager or supervisor experience within the last 3 years.

* Current Registered Nurse (R.N.) license.

* BSN degree

Preferred Skills:

CM certification preferred

Master's degree or equivalent is preferred

Special Skills:

* Excellent customer service and presentation skills are a must

* Strong interpersonal and written communication skills are essential

* Demonstrated ability to apply analytical and problem solving skills

* Demonstrated ability to manage multiple tasks or projects effectively

* Ability to work independently as needed with a high degree of detail orientation

* Capable of planning and organizing projects with short notice

* Ability to work efficiently in a fast-paced environment with changing priorities

Founded in 1956, Bakersfield Memorial Hospital was created to meet the needs of our community, and has grown from a small local facility to a large regional hospital serving all of Kern County.

Today, we have more than 400 general acute beds, nearly 50 intensive care and cardiovascular recovery units, 13 state-of-the-art surgical suites, a full-service ER with nationally certified Stroke Center and the Central CA Heart Institute.

In addition, we offer newly expanded birthing suites, a family care center, a 31 bed NICU, a 20 bed Pediatric unit, a full complement of diagnostic laboratory and imaging services and an outpatient surgery center. Other programs of note include our Women's Services and a Center for Wound Care and Hyperbarics.

The Dignity Health Bakersfield Infusion Center is an off-site center that is located on the campus of a large Oncology practice. Consisting of 50 chairs, we provide chemotherapy and non-chemotherapy infusion and injection services to the surrounding community.
