Social Worker MSW - Supplemental

The Social Workeris responsible for psychosocial assessments of high-risk patients of all ages, in all areas of patient care.The Social Workerworks with the local facilities and community agencies upon request to assist patients with post discharge plans.

REQUIREMENTS:

Previous social work experience

MSW

Social work skills, including assessment, counseling, crisis intervention and referral and discharge planning

Public relation skills to maintain an effective liaison between the hospital and community referral resources

PREFERENCES:

Experience in a healthcare clinic
