Now we're looking to reinforce our team with people who are decisive, brilliant and built for speed.  Join with us and start doing your life’s best work.Primary Responsibilities:Engage members face to face to educate about their healthReview available member service records and relevant documentation (e.g., utilization history, functional level, stratification information, current plan of care)Conduct comprehensive non-clinical member assessment that includes bio-psychosocial, functional, and behavioral health needsUtilize interviewing techniques and active listening to collect and retain member information and incorporating responses as they are presented to complete assessmentIdentify member service needs related to health concernsIdentify urgent member situations and escalate to next level when necessaryWork with clinical staff to engage the member in participating in the assessment process and collaboratively developing the plan of care based on the member’s individual needs, preferences, and objectivesWork with members to identify barriers to accessing care and achieving person-centered goals, as well as work to close gaps in careIdentify member support systems available and incorporate into plan of careReview plan benefits and identify appropriate programs and services based on heath needs and benefitsWork with clinical staff to integrate health care and service needs into a plan / recommendations for member care and serviceTransfer the member to clinical staff to manage clinical needs, as identifiedWork collaboratively with the interdisciplinary care team to ensure an integrated team approachCollaborate with member to create solutions to overcome barriers to achieving healthcare goals-Identify relevant community resources available based on member needsRefer members to appropriate programs and servicesFacilitate member choice of preferred providerAdvocate for individuals and communities within the health and social service systemsRole assists in coordinating health services and does not write care plansHelp members connect with transportation resources and give appointment reminders in special circumstancesTransporting members is strictly prohibitedFollow-up with members via phone calls, home visits and visits to other settings where members can be found

Requirements

Required Qualifications: High School Diploma or GED 1+ years of Medicaid / Medicare experience or 1+
years of experience as a Certified Community Health Worker or 1+ years of experience
working in the community / community programs of 1+ years of experience working as a
case manager1+ years of telephonic customer service experience Knowledge of Medical Terminology Ability to create, copy, edit, send, and save utilizing Microsoft Word, Microsoft Excel, and Microsoft Outlook Must have reliable transportation and required insurance Preferred Qualifications: Associate's Degree or higher Completion of Community Health Worker Training program Previous employment in a medical office, hospital or clinical or other provider environment Bilingual Careers at UnitedHealthcare Community & State.
