Social Worker/Case Manager - Full-time / Part-time

Aerotek is currently looking to hire a social worker for a LTC client in the Baltimore, MD area. Candidates will be responsible for:

• Providing clinical social work services or psychosocial assessments to patients/residents and families, regarding the following common problems: adjustment to instituitional living, financial concerns, family relationships, difficulty understanding/accepting medical/diagnoses/procedures/treatments, changes relating to aging, powerlessness, discharge planning and counseling related to death and dying.

Qualifications:

• Preferably an LCSW, but would be open to LGSW.
• Case Manager RNs would be acceptable as well.
