The ACO (Accountable Care Organization) Care Coordinator is responsible for assisting the Nurse Care Manager with coordinating care for ACO beneficiaries. The ACO Care Coordinator will explain and define the ACO program to Medicare beneficiaries. They will assist in the coordination of care efforts which include: conduct safety, depression screenings, facilitate post discharge appointments for the beneficiary, assist with transportation as needed, and track the patient's compliancy with physician's orders. Under the guidance of the ACO Nurse Care Manager, the Care Coordinator will enroll patients into the ACO by conducting basic health screenings, patient education regarding urgent care, hospice, flu vaccination clinics and other issues related to chronic disease care. 1. Assists ACO beneficiaries with arranging services such as home health, DME, physician appointments, specialist appointments, and transportation and follows up with beneficiaries to ensure arranged services are received. 2. Identifies patients from Passport eligibility verification list that have been admitted to a facility and coordinate the discharge plan. 3. Requests admission and discharge records for coordination of care from discharging facilities. 4. Notifies providers of patients' admission and health status and verifies patient follows up post discharge. 5. Conduct basic health screenings, including safety and depression screenings for high risk patients. 6. Follow up daily with patient discharges and conduct post discharge calls and well calls to ACO beneficiaries within the assigned timeframe. 7. Demonstrates the ability to follow through with requests, sharing of critical information, and getting back to individuals in a timely manner. 8. Demonstrates honesty and integrity in everyday activities. 9. Protects privacy for both beneficiaries and employees; ensuring all personal health information is kept confidential and HIPAA compliant i.e., (sensitive papers, charts, and reports are not in view of the public). 10. Treats beneficiaries and their families with respect and dignity. 11. Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of beneficiaries and their families. Functions as liaison between administration, beneficiaries, physicians, and other healthcare providers. 12. Interacts professionally with other healthcare service providers. 13. Communicates appropriately and clearly to management, co-workers, and physicians. 14. Consults other departments as appropriate to provide for an interdisciplinary approach to the Beneficiary's needs. 15. Ability to be flexible and adaptable to changes in the daily workflow. 16. Participates in the efficient, effective, and responsible use of resources such as supplies and equipment. 17. Maintains all reference material that is provided by the supervisor, manager or director. 18. Knows and follows the Employee Handbook policies and procedures. 19. Follow the assigned workflow. All assigned work must be completed by the end of business day in order to maintain customer service to ACO beneficiaries. 20. Performs other duties and special projects as required. 21. Performs other duties as assigned by the ACO Supervisor.

Qualifications

  1. High school graduate required; Associate in Arts degree preferred. 2. Two years of related medical experience with patient contact preferred. 3. Bilingual in Vietnamese preferred 4. Typing 40 words per minutes with accuracy. 5. Knowledge of computers, faxes, printers and all other equipment. 6. Proficient in Microsoft applications, Excel, Word and Outlook. 7. Ability to discuss and be responsible with confidential matters. 8. Ability to work in a multi-task, high stress environment. 9. Flexible and adapts to process changes.
