The Integrated Supportive Care (ISC) social worker will explain the benefits of the ISC program to patients and families in a culturally sensitive and linguistically competent way. Position will focus on assisting eligible target patients to enroll in the ISC program to provide them the supportive care they need in the community to avoid future preventable health crises related to their serious and chronic illnesses resulting in a medically inappropriate ER visit or hospital admission. Social worker will assess patient's psycho-social needs, assist patient with advance healthcare directive and meet their supportive care needs.

Qualifications:

Master's degree in social work required. Bilingual (English/Armenian or English/Spanish -- verbal and written for both) for effective communications with patients and families of all social and economic levels from a diverse cultural and ethnic population. Must be a motivated and organized team player, teachable and eager to learn new ways to provide psycho-social and supportive care across different care settings. Have empathy and can establish rapport with people dealing with a life-limiting illness. Must be able to work independently, be outcomes-driven and can travel to meet with patients and families in different care settings. Must have good work and personal ethics, and present a professional attitude.

*This is a grant funded position. The duration and availability of this position will be dependent on the existence of the grant funding.
